The document must clearly show (1) the full name of the individual or registered business name, (2) the complete physical address (street, postal code, city, country), and (3) the document issue date. The document must be submitted as a PDF or clear color scan. Screenshots, handwritten documents, mobile phone bills (when not the proof of residence), and bills older than 3 months are not acceptable.

Example: A PDF of an Iberdrola electricity bill dated within the last 90 days, addressed to the customer at a Spanish residential address, for a Spain phone number order. The bill must show the customer's name, full service address, and the issuer's logo and billing date.

A signed Letter of Intent (LOI) executed by the end user or their authorized representative within 1 month of the order being placed. The LOI must clearly state the customer's intent to obtain or port the phone number(s), must list the specific numbers covered, and must bear the signing date and the authorized signature. LOIs older than 1 month from the date of the order are not acceptable; a fresh LOI must be obtained.

Example: A PDF Letter of Intent signed by the authorized representative within the past 30 days, listing the phone numbers being requested or ported, the customer's legal name, the signing date, and the representative's signature.

Contact information for the business or end user, including: (1) the name of the contact person (full given name and family name), (2) the business name when the end user is a business (registered legal name where applicable; may be omitted or stated as 'N/A' for individual end users), and (3) one or more contact phone numbers where the business or end user can be reached. Contact person name and at least one phone number are mandatory; business name is required only when the end user is a business entity. Phone numbers should be in international E.164 format where possible.

Example: Contact: Maria García López | Business: Bloom & Petal Florist Ltd. | Phone: +34 612 345 678. For individual end users: Contact: John Smith | Business: N/A | Phone: +1 (555) 123-4567.

A valid, real-world physical address located in the same country as the phone number being purchased. The address must include street name and number, postal or ZIP code, city, region or province (where applicable), and country. The address does not need to match the specific city or area code of the number; any address within the national borders is acceptable. P.O. boxes, virtual office addresses, mailbox-only services, and addresses outside the country of the phone number are not acceptable. The address should match the address shown on supporting identity or business registration documents where those are also required.

Example: For a Spanish phone number: Calle de Alcalá 42, 28014 Madrid, Spain. For a German phone number: Friedrichstraße 123, 10117 Berlin, Germany. The address must be a deliverable physical location, not a P.O. box or virtual mailbox.

A valid email address for the end client (the natural person or business that will use the phone number), used for compliance follow-up. The submitted value must be a syntactically valid email address (max 150 characters) and should reach the end client directly. Generic or shared role-based addresses (e.g., info@, contact@) are acceptable but a personal address tied to the named end client is preferred.

Example: maria.garcia@bloomandpetal.com or contact@acme-coffee.de. The address must be syntactically valid (matching the standard email format) and should be a working address.

A copy of the company registration or incorporation certificate issued by the government registry of the same country as the phone number being purchased. The document must show the registered legal company name (matching the order), the unique company registration or incorporation number, the date of registration, the registered business address, and the issuing authority. Submitted as a clear color scan or original PDF in the local language or English, dated within the past 3 months unless a different timeframe applies at the country level.

Example: For a Spanish phone number: a Certificado de Inscripción from the Spanish Registro Mercantil. For an Italian phone number: a Visura Camerale from the Italian Camera di Commercio. The document should show the legal company name, registration number, registration date, and registered office address.

A clear color copy of a valid, unexpired government-issued identity document for the end user (for personal accounts) or the authorized representative of the company (for business accounts). Acceptable documents are the biographical data page of a passport, a national identity card (front and back), or, where local rules permit, a driver's license. The copy must legibly show the holder's full legal name, photograph, date of birth, document number, issuing country and authority, issue date, and expiration date. The document must not be expired as of the submission date and must be a color scan or original PDF. Black-and-white copies, photographs at extreme angles, and screenshots of digital wallet IDs are not acceptable.

Example: A color PDF of the biographical data page of a U.S. passport, or a color scan of both sides of a French Carte Nationale d'Identité. The holder's name on the document must match the name provided in the order's customer or representative fields.

Identity verification document, with type depending on end user. For BUSINESS end users: a certificate of business registration or incorporation, dated within the past 3 months, demonstrating the place of business. The certificate must show the registered legal company name (matching the order), the registration number, the registered business address, and the issuance date. For PERSONAL end users: a government-issued ID such as a national ID card, permanent resident card, or passport. The ID must clearly show the holder's full name, photograph, document number, and a valid (unexpired) expiration date.

Example: Business: a Companies House Certificate of Incorporation or Handelsregisterauszug, dated within the past 90 days. Personal: a color scan of a passport biographical page, both sides of a national ID card, or a permanent resident card with a valid expiration date.

A physical address that matches the geographic area covered by the phone number's area code. The address must include street name and number, postal/ZIP code, city, and country, and the city/region of the address must fall within the geographic area assigned to the DID's area code. Addresses outside the area code's geographic region are not acceptable for this requirement; a different (broader, national-level) address requirement must be selected if the customer cannot provide a matching local address.

Example: 311 W Superior St, Chicago, IL 60654 (matches Chicago area code 312). For a +44 20 (Greater London) DID: an address within Greater London such as '60 Holborn Viaduct, London EC1A 2FD'. The address must lie within the area code's geographic boundaries.

The bill must clearly show the customer's full name (matching the order), the complete physical address (street, postal code, city, country), the issue date, and the issuing utility provider. Example: A PDF or color scan of an electricity bill (e.g., from EDF, E.ON, Iberdrola, or similar regional provider) addressed to the customer, with an issue date within the past 90 days, showing the customer's name, full address, and provider details.

A jurisdiction-specific Registration Form for phone numbers in Georgia (the country, not the U.S. state). The form must be completed and signed by the customer or authorized representative for compliance with Georgian telecommunications regulations. The completed form must be returned with all required fields populated, the appropriate signature applied, and (where applicable) a company stamp or seal. Customers should request the current template from the Telnyx Numbering team if they have not received one.
