A signed Letter of Intent (LOI) executed by the end user or their authorized representative within 1 month of the order being placed. The LOI must clearly state the customer's intent to obtain or port the phone number(s), must list the specific numbers covered, and must bear the signing date and the authorized signature. LOIs older than 1 month from the date of the order are not acceptable; a fresh LOI must be obtained.

Example: A PDF Letter of Intent signed by the authorized representative within the past 30 days, listing the phone numbers being requested or ported, the customer's legal name, the signing date, and the representative's signature.

Contact information for the business or end user, including: (1) the name of the contact person (full given name and family name), (2) the business name when the end user is a business (registered legal name where applicable; may be omitted or stated as 'N/A' for individual end users), and (3) one or more contact phone numbers where the business or end user can be reached. Contact person name and at least one phone number are mandatory; business name is required only when the end user is a business entity. Phone numbers should be in international E.164 format where possible.

Example: Contact: Maria García López | Business: Bloom & Petal Florist Ltd. | Phone: +34 612 345 678. For individual end users: Contact: John Smith | Business: N/A | Phone: +1 (555) 123-4567.

A document evidencing the customer's physical address, issued within the past 3 months. The address may be located in any country in the world; this requirement does NOT impose a country match with the phone number. Acceptable documents include utility bills (electricity, water, gas, internet, landline telephone), bank statements, tax notices, government correspondence, or insurance documents. The document must clearly show (1) the full name of the individual or registered business name, (2) the complete physical address, and (3) the document issue date. Mobile phone bills, screenshots, and bills older than 3 months are not acceptable.

Example: A PDF of a utility bill (e.g., from any electricity, water, gas, internet, or landline provider in the customer's country) addressed to the customer at their physical address, with an issue date within the past 90 days.

A valid, real-world physical address located anywhere in the world. Must include street name and number, postal code (where applicable), city, region or province (where applicable), and country. The address provided must correspond to a validated physical address (e.g., as enriched via address validation in the Telnyx system). P.O. boxes, virtual office addresses, and mailbox-only services may not be acceptable depending on the order's downstream country requirements; a deliverable physical location is preferred.

Example: 123 Main St, Brooklyn, NY 11217, US. Or: 60 Holborn Viaduct, London EC1A 2FD, United Kingdom. The address can be in any country in the world but must be a real, deliverable physical location.
