A signed Letter of Intent (LOI) executed by the end user or their authorized representative within 1 month of the order being placed. The LOI must clearly state the customer's intent to obtain or port the phone number(s), must list the specific numbers covered, and must bear the signing date and the authorized signature. LOIs older than 1 month from the date of the order are not acceptable; a fresh LOI must be obtained.

Example: A PDF Letter of Intent signed by the authorized representative within the past 30 days, listing the phone numbers being requested or ported, the customer's legal name, the signing date, and the representative's signature.

Contact information for the business or end user, including: (1) the name of the contact person (full given name and family name), (2) the business name when the end user is a business (registered legal name where applicable; may be omitted or stated as 'N/A' for individual end users), and (3) one or more contact phone numbers where the business or end user can be reached. Contact person name and at least one phone number are mandatory; business name is required only when the end user is a business entity. Phone numbers should be in international E.164 format where possible.

A document evidencing the company's physical address that matches the registered address shown on the Business Registration Certificate provided. Acceptable documents include utility bills (electricity, water, gas, internet, landline telephone), commercial lease agreements, bank statements addressed to the business, or government correspondence. The document must show the registered business name, the complete physical address (street, postal code, city, country) exactly matching the Business Registration Certificate, and an issue date within the past 3 months. Mobile phone bills, screenshots, and personal addresses are not acceptable.

Example: An electricity utility bill addressed to 'Acme GmbH' at 'Friedrichstraße 123, 10117 Berlin, Germany', with an issue date within the past 90 days, where the address exactly matches the address shown on Acme GmbH's Handelsregisterauszug.

A copy of the company registration or incorporation certificate for the company placing the order. The certificate does not need to be issued in the same country as the phone number being ordered. The document is acceptable as long as it is issued by a recognized company registrar in the country where the company is registered and is written either in English or in the local language of the country of company registration. The document must show the registered legal company name matching the order, the unique registration or incorporation number, the date of registration, the registered business address, and the issuing authority. Submitted as a clear color scan or original PDF.

Example: A Companies House certificate (UK, in English), a Handelsregisterauszug (Germany, in German), or an Extrait Kbis (France, in French), each showing the company's legal name, registration number, registration date, and registered office address.

A valid, real-world physical address located anywhere in the world. Must include street name and number, postal code (where applicable), city, region or province (where applicable), and country. The address provided must correspond to a validated physical address (e.g., as enriched via address validation in the Telnyx system). P.O. boxes, virtual office addresses, and mailbox-only services may not be acceptable depending on the order's downstream country requirements; a deliverable physical location is preferred.

Example: 123 Main St, Brooklyn, NY 11217, US. Or: 60 Holborn Viaduct, London EC1A 2FD, United Kingdom. The address can be in any country in the world but must be a real, deliverable physical location.

A clear color copy of a valid, unexpired government-issued identity document for the end user (for personal accounts) or the authorized representative of the company (for business accounts). Acceptable documents are the biographical data page of a passport, a national identity card (front and back), or, where local rules permit, a driver's license. The copy must legibly show the holder's full legal name, photograph, date of birth, document number, issuing country and authority, issue date, and expiration date. The document must not be expired as of the submission date and must be a color scan or original PDF. Black-and-white copies, photographs at extreme angles, and screenshots of digital wallet IDs are not acceptable.

Example: A color PDF of the biographical data page of a U.S. passport, or a color scan of both sides of a French Carte Nationale d'Identité. The holder's name on the document must match the name provided in the order's customer or representative fields.
