A signed Letter of Intent (LOI) executed by the end user or their authorized representative within 1 month of the order being placed. The LOI must clearly state the customer's intent to obtain or port the phone number(s), must list the specific numbers covered, and must bear the signing date and the authorized signature. LOIs older than 1 month from the date of the order are not acceptable; a fresh LOI must be obtained.

Example: A PDF Letter of Intent signed by the authorized representative within the past 30 days, listing the phone numbers being requested or ported, the customer's legal name, the signing date, and the representative's signature.

A working contact phone number for the authorized person associated with the order, used for compliance follow-up. The submitted value should be a valid telephone number, preferably in international E.164 format (with country code prefix). Landlines and mobile numbers are both acceptable. Generic shared lines (main switchboard) may be acceptable but a direct contact for the named authorized person is preferred.

Example: +34612345678 (Spain mobile, E.164), +442071234567 (UK landline, E.164), or 0123456789 (national format where international format is not available). The submitted number should reach the authorized representative directly.

A valid, real-world physical address located anywhere in the world. Must include street name and number, postal code (where applicable), city, region or province (where applicable), and country. The address provided must correspond to a validated physical address (e.g., as enriched via address validation in the Telnyx system). P.O. boxes, virtual office addresses, and mailbox-only services may not be acceptable depending on the order's downstream country requirements; a deliverable physical location is preferred.

Example: 123 Main St, Brooklyn, NY 11217, US. Or: 60 Holborn Viaduct, London EC1A 2FD, United Kingdom. The address can be in any country in the world but must be a real, deliverable physical location.

The full legal name of the company associated with the phone number, exactly as registered with the relevant national company registry. The name must include any legal suffix such as 'LLC', 'Ltd.', 'GmbH', 'S.A.', 'S.r.l.', or equivalent. Maximum 150 characters, minimum 1 character. Trade names and informal abbreviations without legal status are not acceptable; the registered legal name is required.

Example: Telnyx LLC, Acme Florist Limited, Bloom & Petal GmbH, or Empresa Ejemplo S.A. The submitted name must match the company's official registration certificate; informal trading names ('Acme', 'Bloom') without the legal suffix are not acceptable.

An official company registration or incorporation certificate issued by the relevant government registry. Required for business end users only. The document must contain key company information including the registered legal name (matching the order), the unique registration or incorporation number, the date of registration, the registered business address, the legal form, and the issuing authority. Submitted as a clear color scan or original PDF in the local language or English. Should be dated within the past 3 months unless a different timeframe applies at the country level.

Example: A Companies House Certificate of Incorporation (UK), a Handelsregisterauszug (Germany), an Extrait Kbis (France), or a U.S. Secretary of State Certificate of Incorporation, showing the company's legal name, registration number, registration date, registered office address, and legal form.
