A valid URL pointing to the active public-facing website of the business associated with the phone number. Required for business end users only. The URL must use http:// or https:// scheme and must resolve to a live website that clearly identifies the business by its registered or trading name. Social media profiles (Facebook, Instagram, LinkedIn pages) are generally not acceptable as a substitute for a primary business website unless the business has no other web presence and this is approved at the country level. Parked domains, 'coming soon' pages, and 404 pages are not acceptable.

Example: https://telnyx.com/ or https://www.example-florist.com. The URL must resolve to a live, working website that visibly represents the business named on the order.

Identity documentation, with the required document depending on whether the number will be registered to a company or to a person. For NUMBERS REGISTERED TO A COMPANY: a company registration certificate is required. For NUMBERS REGISTERED TO A PERSON: a passport, ID, or driver's license copy is required. The submitted document must be a legible copy with text and security features readable; the company certificate must show legal name, registration number, and registered address; the passport, ID, or driver's license must show full name, photograph, document number, and a valid (unexpired) expiration date.

Example: Company use case: a Companies House Certificate. Personal use case: a legible scan or photo of the individual's passport biographical page, national ID card, or driver's license. Color is preferred but grayscale is acceptable provided all required fields and security features are clearly readable.

Contact information for the business or end user, including: (1) the name of the contact person (full given name and family name), (2) the business name when the end user is a business (registered legal name where applicable; may be omitted or stated as 'N/A' for individual end users), and (3) one or more contact phone numbers where the business or end user can be reached. Contact person name and at least one phone number are mandatory; business name is required only when the end user is a business entity. Phone numbers should be in international E.164 format where possible.

A document evidencing the customer's physical address, issued within the past 3 months. Acceptable documents include utility bills (electricity, water, gas, internet, landline telephone), invoices from the current telecommunications provider, or insurance policy documents. The document must clearly show (1) the full name of the individual or registered business name (matching the order), (2) the complete physical address (street, postal code, city, country), and (3) the document issue date. Mobile phone bills, screenshots, handwritten documents, and bills older than 3 months are not acceptable.

Example: A PDF of the latest invoice from the customer's current phone provider, addressed to the end user at their registered address, with an issue date within the past 90 days; or an electricity utility bill with the same.

A free-text description, in English, explaining how the phone number will be used in the customer's business operations. The description must (1) name the business or end customer, (2) describe the specific calling activity (e.g., inbound customer support, outbound sales, two-factor authentication, IVR, conferencing), and (3) explicitly state whether the number will be sub-allocated to a third party. Sub-allocation means re-assigning or sub-licensing the number to another business or end user. If sub-allocation applies, the customer must name the sub-allocated party and describe their use case as well. Vague responses such as 'business use' or 'voice services' on their own are not acceptable.

Example: Example A (no sub-allocation): 'This number will be used as the main inbound customer service line for Bloom & Petal Florist, a retail flower shop in Madrid, and will not be sub-allocated to any third party.' Example B (with sub-allocation): 'This number will be sub-allocated to Acme Healthcare Ltd. for use as their appointment reminder outbound dialing line.'

A valid, real-world physical address located anywhere in the world. Must include street name and number, postal code (where applicable), city, region or province (where applicable), and country. The address provided must correspond to a validated physical address (e.g., as enriched via address validation in the Telnyx system). P.O. boxes, virtual office addresses, and mailbox-only services may not be acceptable depending on the order's downstream country requirements; a deliverable physical location is preferred.

Example: 123 Main St, Brooklyn, NY 11217, US. Or: 60 Holborn Viaduct, London EC1A 2FD, United Kingdom. The address can be in any country in the world but must be a real, deliverable physical location.
